[ Summary ] The Japanese population is aging, and social security and health care costs continue to increase. To distribute the finite medical and nursing care resources effectively, it is also necessary to use efficient medical technologies that balance costs and benefits. Medical economic evaluation is useful for determination of efficiency. In this paper, we discussed the methodology and research on the cost-effective prevention and treatment of osteoporosis. The aim of osteoporosis treatment is fracture prevention. There are many reports on the cost-effectiveness of drugs that show evidence of prevention of fragility fractures. It is cost-effective to use preventive treatment for osteoporosis, based on the understanding of the characteristics of each drug and consideration of the age of the patient and individual risk of fracture. In addition, interventions to increase the low rate of osteoporosis treatment after occurrence of fragility fractures have been reported to be cost-effective.
