[ Summary ] Diabetic patients are at higher risk of fractures than wold be expected by their measured bone mineral density (BMD). Poor bone quality is the most likely and explainable cause for the elevated fracture risk in this population. Diminished bone quality may be caused by reduced material and structural properties of bone such as accumulation of advanced glycation end-products in bone collagen, impaired bone formation, and thinner as well as porous shape of cortical bone. We need to carefully select hypoglycemic agents to avoid increased risk of fractures.
