[ Summary ] In the present review, we discuss epidemiological indices such as the prevalence and cumulative incidence of osteoporosis (OP) in Japan, determined using the information of a large-scale population-based cohort study entitled Research on Osteoarthritis/osteoporosis Against Disability (ROAD). First, the prevalence of OP at the lumbar spine L2-4 was 3.4 % in men and 19.2 % in women ; the OP prevalence at the femoral neck was 12.4 % in men and 26.5 % in women. The annual cumulative incidences of OP at the lumbar spine L2-4 and the femoral neck were 0.76 %/y and 1.83 %/y, respectively. Serum 25-hydroxyvitamin D levels could predict the occurrence of OP at the femoral neck within 3 years. Moreover, the presence of OP was significantly associated with short-term sarcopenia occurrence.
