[ Summary ] The Revised 2012 Diagnostic Criteria for Primary Osteoporosis and the Criteria for Initiation of Pharmacological Treatment as part of the 2011 Guidelines for Prevention and Treatment of Osteoporosis have been combined to provide the cornerstone for the management of osteoporosis in Japan.
The former addresses several issues that remained unresolved in the earlier 2000 criteria, ensures consistency with global standards of care, and reflects the new evidence accumulated during the intervening 12 years. On the other hand, the latter represents an updated version of the 2006 guidelines, which provided, for the first time, criteria for initiating pharmacological treatment in osteoporosis based on fracture risk. However, this time, the focus is mainly on proximal femoral fractures and vertebral fractures. Moreover, specific FRAX® cut-off values for the indication for osteoporosis screening are presented, which is of particular interest in encouraging aggressive therapeutic interventions in patients at high risk of osteoporotic fractures, particularly in those at risk of proximal femoral fractures, which still remain less well controlled in Japan.
