[ Summary ] The definition of CKD-MBD is applicable to both adults and children. Growth retardation is one of the characteristic clinical manifestations seen in children with CKD. However, optimal control of both growth retardation and MBD is still difficult in clinical settings. New drugs including calciumu-free phosphate binders and calcimimetic agents have not been approved for use in children. In order to minimize complications related to growing skeletons and to prevent extraskeletal calcification, paticular attention must be paid to alterations of MBD in children with CKD. The main clinical manifestations in children with CKD are growth retardation, skeletal deformities, and cardiovascular calcifications. Because patients are usually asymptomatic, insufficient attention is often paid to progressive alterations in bone and mineral homeostasis. Therefore, prevention should always be the primary objective in children with CKD.
