[ Summary ] The pathophysiology of CKD-MBD in kidney transplant recipients (KTRs) is multifactorial. It never starts after kidney transplantation. However, it emerges in the pre-transplant dialysis period. Given that we have no established treatment for CKD-MBD in KTRs in the posttransplant period, the management of CKD-MBD from the dialysis period through transplantation should certainly be regarded as a practical and effective measure. In this article, the possible mechanisms of CKD-MBD in KTRs and the new Japanese guidelines for CKD-MBD in KTRs are introduced.
