[ Summary ] Achievement of successful long-term kidney graft survival through improvements in immunosuppressive agents has highlighted the importance of management of various complications after transplantation. Bone fractures are critical events for kidney transplant recipients. Posttransplant bone disease, or CKDT-MBD (transplant chronic kidney disease-mineral and bone disorder), is prevalent as a hotbed of fractures. Because the pathogenesis of CKDT-MBD is very complicated, a standardized treatment is not available. In this review, a number of therapeutic strategies reported recently are introduced along with projected treatment effects. Although knowledge related to the 'posttransplant' field is accumulating, the number of randomized controled trials is insuffi cient to prepare treatment guidelines, especially for CKDT-MBD.
