[ Summary ] Serum magnesium concentrations are normal in patients with early renal failure. In the advanced stages of renal failure, hypermagnesemia usually occurs due to reduced urinary magnesium excretion. However, the existence of intracellular magnesium concentrations in various tissues is controversial. It must be asked, 'what is the association of serum magnesium concentrations to chronic renal failure ?' Studies of magnesium requirements for healthy men and women have been done estimating magnesium balance connected with magnesium intake minus fecal and urinary magnesium excretion. Patients with chronic kidney disease (CKD) are usually malnourished because of poor appetite, nausea, and vomiting. Assessment of magnesium balance in CKD patients is essential in considering magnesium metabolism associated with CKD. Recent human genetic studies on magnesium metabolism are also introduced.
