[ Summary ] Reference interval (RI) for serum total magnesium (SMg) level exists in narrow range. When SMg levels exceed the upper limit of RI, levels under 4.9 mg/dl are considered to be in the therapeutic range and levels over 4.9 mg/dl are considered to be in the critically ill range. The latter occurs when Mg is given in some situations such as renal failure and/or diminished bowel peristalsis. Serious symptoms such as bradycardia, hypotension and cardiac arrest may occur inconjunction with those conditions. Treatments for Mg overloads include removal of Mg load and infusion with calcium salts. However, in some cases, vasopressors, haemodialysis and transvenous cardiac pacing are required.
