[ Summary ] The primary outcome of the JSDT clinical practice guidelines for secondary hyperparathyroidism in dialysis patients is to improve the life expectancy of dialysis patients. These guidelines set a priority on maintaining serum calcium and phosphorus in adequate levels rather than serum PTH levels. The target levels of serum intact PTH are between 60 and 180 pg/ml and lower than that set by the K/DOQI 2003 guidelines. Around the world, the KDIGO has introduced new global guidelines for CKD-MBD. In an era of global guidelines, the purpose of local guidelines will need to be discussed.
