[ Summary ] Phosphate is excreted nearly exclusively from the kidnies. Therefore, hyperphosphatemia is frequently seen in patients with chronic renal failure. Hyperphosphatemia is one of the main factors in the development of secondary hyperparathyroidism. Hyperphosphatemia is also a main cause of ectopinc calcification, particularly vascular calcification, which significantly affects all-cause and cardiovascular mortality. In our studies, it was demonstrated that hyperphosphatemia is a significant, independent factor, associated with the presence or severity of both intimal and medial calcification of the arteries of hemodialysis patients. Furthermore, hyperphosphatemia is demonstrated to be a significant, independent factor associated with arterial wall thickness in these patients. Hyperphosphatemia should be rigorously controlled in uremic patients.
