[ Summary ] The strongest point concerning the secondary hyperparathyroidism guideline in Japan is that the highest priority must be given to improvements in mortality rates. It is of great significant that this guideline preceded the concept of “Chronic Kidney Disease-Mineral and Bone Disorder (CKD-MBD)”, as proposed by the Kidney Disease Improving Global Outcomes (KDIGO) study. However, evidence related to Japanese people is grossly insufficient, and problems related to dialysate calcium concentrations, bone disease and PTH measurement still remains unsolved. More consideration must be given to both the verification related to this guideline and treatment for these patients.
