[ Summary ] It has been shown that fibroblast growth factor (FGF) 23 not only has important roles in the development of hypophosphatemic rickets / osteomalacia but also is a physiological humoral factor regulating phosphate and vitamin D metabolism. Investigation of FGF23 action and FGF23 levels in animal models and measurement of FGF23 concentrations in patients with hyperparathyroidism and hypoparathyroidism indicated that calcium and parathyroid hormone in addition to phosphate and 1, 25-dihydroxyvitamin D appeared as possible candidates for being regulatory factors for production and serum level of FGF23. However, there still remain many unanswered questions about the mechanisms of FGF23 regulation and production. Further studies are necessary to clarify these issues.
