[ Summary ] Several metabolic bone markers are emerging as routine laboratory tests to evaluate bone metabolism without bone biopsy specimens. Among them, bone alkaline phosphatase (BAP), one of bone formation markers reflecting osteoblastic activity, has been established as a clinically useful serum marker since it can be immunologically measured using commercially available kits including monoclonal antibodies specific to BAP. Serum BAP is now a powerful tool for clinical management of osteoporosis and other metabolic bone diseases and cancer bone metastases. Several clinical investigations have suggested that BAP is also useful to evaluate the pathophysiology of renal osteodystrophy.
