[ Summary ] Recently, with the development of ultrasonography techniqus, it has become possible to directly inject various drugs into the parathyroid gland in patients with secondary hyperparathyroidism (2°HPT). First, percutaneous ethanol injection therapy (PEIT) was successfully performed and then PEIT was reported to be as a effective treatment as PTx. However, several problems and/or complications with PEIT have been reported, so as an alternative to PEIT, the direct injection of maxacalcitol, a powerful calcitriol analogue, into the parathyroid gland has been developed. Percutaneous maxacalcitol injection therapy (PMIT) brings about not only the marked suppression of PTH, but also the decrease in parathyroid volume, that is attributable to the induction of apoptosis resulting from high concentrations of maxacalcitol. PMIT is now one of the effective and safe tools for the treatment of 2°HPT.
