[ Summary ] As the population ages, the proportion of elderly patients with end-stage renal failure leading to the maintenance dialysis introduction is high. It is clear that the elderly are at high risk for chronic kidney disease (CKD) and end-stage kidney disease (ESKD). Also, in acute kidney injury( AKI), the incidence of elderly people is higher than in young people. AKI is also important as a factor in exacerbating existing CKD. This is why consideration for prevention is important. There are many complications in the elderly, and even with treatment for AKI, the therapeutic effect of the blood purification method may be limited. In the case of indication for blood purification therapy, considerations different from non-elderly are also required. It is necessary to discuss the wishes of patients and families in particular (shared decision making) and consideration from an ethical point of view is increasingly important.
