[ Summary ] Maintenance of residual renal function (RRF) after initiation of hemodialysis is essential for both purification and ultrafiltration; however, RRF is more likely to decrease in hemodialysis patients than in continuous ambulatory peritoneal dialysis patients. In general, an increase in hemodialysis dose and appropriate ultrafiltration removal are needed to maintain RRF. It is important to remove water slowly to avoid changes in blood pressure; therefore, various methods should be used for evaluation of body fluid status, including blood volume monitoring. Furthermore, the use of nonsteroidal anti-inflammatory drugs and contrast agents, as well as the possibility of cholesterol embolism and infection should be addressed when considering RRF in hemodialysis patients.
