[ Summary ] In pediatric patients with end-stage renal disease, it is essential to prepare a lifelong treatment plan for chronic renal failure to transition from pediatric to adult medical care. While opting for renal replacement therapy (RRT) in children, it is necessary to comprehensively consider age, physique, if a living kidney donor is available, circumstances of the family, and enrollment and advancement of the patient. In this article, we discuss the selection of the type of RRT in Japanese children, reasons why peritoneal dialysis and preemptive kidney transplantation are selected for children, reasons why hemodialysis is difficult, preparation and considerations for RRT, dialysis initiation criteria, and the preemptive cadaver donor kidney transplantation system in Japan.
