[ Summary ] A pediatric patient with a chronic disease requires a smooth transition from pediatric to adult medical care. An outpatient nursing department was started for patients 15 years of age or older to provide support during this transition. Here, a patient is provided age appropriate health literacy, help with maintenance of their mental health and coordination with their support system.
It is important for nurses in the outpatient department to develop a relationship with the patient and to help them become independent in their medical decisions and social life. This is necessary for such patients to transition into self-managed medical care during adulthood.
