[ Summary ] More aggressive rehabilitation has become applicable to patients since the HD procedure was approved in convalescent stage-rehabilitation wards in April 2012. In the wards, specialists, as a team, provide intensive rehabilitation programs for patients for function recovery and discharge from the hospital to their homes (especially for their ADL). The most important point is for the patient to be self-supportive, that is, take a meal and use the lavatory by himself. In order to realize this, the specialists need to hold meetings frequently, to share detailed information about patients and to maintain close contact among the team. It is also important to be ready for risks that patients may be faced with, to ensure their safety and security. In addition, it is necessary for the team to pay attention to disuse syndrome, which is often observed apart from the original disease the patient is suffering from.
