[ Summary ] The field of dialysis care has become more complicated over time. Therefore, it has become easier for human errors to occur. While measures to prevent recurrence of many accidents have been devised and practiced on site, errors related to dialysis therapy have unfortunately not diminished. In this paper, an example is given of an error in a blood purifi cation column at the Kyoto University Hospital. Changes in D port shape for plasma separators as a measure to prevent the recurrence of such accidents, and barcode medication administration for patients are reviewed. Problems with dialysis patient management systems and the usefulness of artificial intelligence are also reviewed.
