[ Summary ] Renal transplantation is expected to enhance the quality of life and life expectancy, and to reduce medical expenses for dialysis. In Japan, however, renal transplantation accounts for less than 5 % of RRT, and data on operable age, transplant outcomes, and surgical expenses is not yet fully understood. Although the rate of graft survival in renal transplantation has improved, 10-30 % of recipients eventually resume dialysis. Even though transplant recipients may return to dialysis, continuous, life-long management of renal failure is required. The aim is to save a life, not a failed graft. At any point, it is necessary to refer the patient for appropriate RRT, depending on the circumstances.
