[ Summary ] It is necessary to resolve troubles related to dialysis machine failure, during treatment. Appropriate strategies to deal with equipment failure must be created and implemented.
When an abnormal situation arises, we must be sure to secure affected machines so they will not be used until repaired. When looking for cause (or causes), it is necessary to list all potential causes, and address them in an appropriate order based on frequency of problem, and all safety issues.
Tools such as the FTA may be used for evaluation of safety. Tools such as risk matrix methods can be used also. It is unusual for multiple independent abnormalities to occur at the same time. As a result, it is necessary to try to find a single cause which can reasonably explain the defect phenomenon for reasonable cause estimation.
