[ Summary ] In order to achieve safe medication use in hospitals, medical personnel have suggested various solutions. However, the number of medication errors is still high. It is important to eliminate medication-related incidents to improve patient safety. Joint Commission International, the global accreditation agency for healthcare organizations in the world, suggests we establish policy for medication management, use and documentation. These policies would guide various healthcare professionals to provide safe medication use. Our experiences when following JCI standards and changes made in hospital operation, especially in the dialysis unit, along with the use of multiple medications (polypharmacy) are discussed.
