[ Summary ] CKD (GFR < 60 mL/min/1.73 m2) is a risk factor for the development of contrast induced nephropathy (CIN). Since the volume of contrast media used is a risk factor for developing CIN, the volume of contrast media should be the minimum necessary to obtain adequate radiographs. It is recommended that patients with CKD who are at high risk of CIN receive isotonic solutions such as physiological saline and sodium bicarbonate solutions intravenously before and after contrast-enhanced examination. Because there is no evidence indicating that hemodialysis decreases the risk of CIN, hemodialysis after contrast exposure for this purpose is not recommended.
In order to prevent nephrogenic systemic fibrosis (NSF), gadolinium contrast media should not be used in patients with stage 4 or 5 CKD, or acute renal failure as a general rule.
