[ Summary ] Growth retardation is a characteristic clinical manifestation observed in children with chronic kidney disease (CKD). However, optimal care to minimize both growth retardation and mineral and bone disorder (MBD) is difficult in clinical settings. Additionally, new drugs including calcium-free phosphate binders and calcimimetic agents have not yet been approved for use in children. The main clinical manifestations observed in children with CKD are growth retardation, skeletal deformities, and cardiovascular calcification. Since pediatric patients are usually asymptomatic, insufficient attention is often paid to progressive alterations in bone and mineral homeostasis. Therefore, in order to minimize complications related to the growing skeleton and to prevent extraskeletal calcification, paticular attention must be paid to alterations of MBD in children with CKD. Furthermore, prevention of these complications should always be the primary objective for treatment of children with CKD.
