In medical team of dialysis, pharmacists can frequently demonstrate their clinical expertise. Pharmacists may design appropriate drug therapies for patients via the early detection of side effects and the confirmation of the pharmacological effects. Intriguingly, aggressive intervention by pharmacists has been reported to improve medication adherence. In addition, pharmacists play an important role by sharing information about medication therapy during dialysis with other professionals such as doctors, nurses, nutritionists, and clinical engineers. In order to fully make use of their expertise, it is necessary for pharmacists to participate in medical team of dialysis actively, and to communicate with patients and medical professionals more closely. |
