[ Summary ] We performed a survey by questionnaire to evaluate the degree of achievement of gathering chronic kidney disease (CKD) information. Our aim was also to provide better CKD prevention programs through nephrologists. Five hundred and six general practice physicians, excluding nephrologists, replied to this questionnaire concerned with CKD medical care in Hyogo prefecture. We analyzed patients in terms of their actual state of health, including renal function markers, composition of medical care and dietary instruction, as well as timing of referral to nephrologists.
