[ Summary ] The main indication for off-line hemodiafiltration (Off-HDF) is dialysis hypotension. On-line hemodiafiltration (OL-HDF) is used for hemodialysis patients with complications in which middle and/or large molecule uremic toxins cause conditions such as dialysis related amyloidosis. The efficiency of HDF is owed to its treatment characteristics. These characteristics include maintenance of plasma osmolality through isotonic substitution fluids, lowered levels of middle and/or large molecule uremic toxins (especially for OL-HDF), improved biocompatibility with dialysis membranes, and suppressed micro-inflammation due to ultrapure dialysis fluids (for OL-HDF only). Clinical evidence has been reported indicating improvements in dialysis hypotention when employing Off-HDF, the improvement of dialysis related amyloidosis and improved patient survival rates due to employing OL-HDF.
