[ Summary ] Health care for renal disease includes care for chronic kidney disease (CKD), renal replacement therapy and treatment for acute kidney injury. In order to provide early detection of and intervention for CKD, a cooperative health care among community network is necessary. A strategic outcome study for chronic kidney disease termed the "Frontier of Renal Outcome Modifications in Japan". was designed to encourage CKD patients to consult their physicians, enhance cooperation between nephrologists and general practitioners, and prevent the progression of kidney disease. A CKD care system which will promote collaboration between medical professionals must be established.
