[ Summary ] It has been clearly demonstrated that kidney transplantation improves the prognosis for end-stage renal failure patients. Preemptive kidney transplantation is closely associated with improved kidney allograft survival rates. Recently, clinical outcomes for ABO -- incompatible kidney transplantation have improved. Therefore, this strategy has become an option for kidney transplantation between husbands and wives. The number of living donors has increased, while the number of cadaver donors remains unchanged in Japan. Because current strategies seem to make it difficult to expand the donor pool, another approach is needed to incentivize living organ donations. These incentives, include legalizing payment of a fair price to donors, as proposed in the USA. In addition, it is essential to improve care for chronic kidney disease occuring in kidney transplant recipients and donors.
