[ Summary ] The number of Japanese patients with chronic kidney disease (CKD) is estimated to be 13 million. CKD patients are at risk of declining renal function and concomitant comorbidity associated with cardiovascular diseases. The Japanese program for detection and classification of CKD was started by the Japanese Society of Nephrology in 2004. Public policy concerning CKD has increased awareness of CKD by medical professionals past 7 years. In spite of efforts being made through CKD initiatives, the prevalence and incidence of CKD in Japan is expected to increase over the next 10 years because of our rapidly aging population and the steadily increasing prevalence of diabetes.
Shortcomings in the present classification of CKD employing eGFR values are well recognized since amount of proteinuria is not used for CKD risk assessment. The revised KDIGO classification is two dimensional and composed of eGFR values and amounts of proteinuria. Causative disease of CKD needs to be noted. Risk assessment will be done accurately by using the new CKD classification, enabling us to treat high risk patients early on and appropriately.
