[ Summary ] Since there is no valid evidence concerning quality of and indications for timing of renal replacement therapy (RRT) for acute kidney injury (AKI), individual institutions conduct RRT in different ways. RRT is not a treatment without complications. Therefore, non-renal indications should not be considered in daily practice because there is no evidence showing that non-renal indications improve outcomes. Although some studies suggest that early initiation of RRT may improve outcomes, these study findings may be affected by different definitions of timing. In the future, the timing of RRT may be decided on by using renal biomarkers. However, at this time, we need to make clinical judgments. The consensus criteria for judgments concerning AKI were created at the beginning of the 21st century. It is hoped that further studies will be conducted to elucidate disease mechanisms and develop treatments.
