[ Summary ] Acute blood purification has become an important treatment method employed in intensive care situations. In order to perform blood purification in a safe and effective manner, the choice of anticoagulants and the amount administered are important factors. Too little anticoagulant can cause coagulation to occur in the circuit and filter system, stopping treatment and reducing effectiveness, while too much anticoagulant can increase the possibility of bleeding. In Japan, numerous facilities, including ours, frequently use nafamostat mesilate. The appropriateness of nafamostat mesilate needs to be reevaluated in consideration of medical economics. The use of anticoagulants from a clinical perspective is examined.
