[ Summary ] The Japanese Society for Dialysis Therapy (JSDT) clinical practice guideline for the management of secondary hyperparathyroidism in chronic dialysis patients have helped clinicians to better manage patients with chronic kidney disease-mineral and bone disorder (CKD-MBD). There are, however, several remaining concerns such as the validity of the JSDT target ranges and the optimal use of novel therapeutic agents that were introduced in Japan after implementation of the JSDT guideline. Under such a situation, the Kidney Disease : Improving Global Outcomes (KDIGO) released clinical practice guideline on the management of CKD-MBD in August 2009. During the development of this guideline, many efforts were made to grade the quality of evidence supporting each recommendation. Given that this guideline will serve as the international standard for the management of CKD-MBD, the revised version of the JSDT guideline should focus on implementation of the practical uses of the KDIGO guideline in Japan and deal with various issues that will arise when this guideline is introduced in Japan.
