[ Summary ] In clinical practice, we frequently experience vascular access (VA) related incidents as follows : 1) difficulty or mistakes in cannula punctures, 2) insufficient extracorporeal blood flow rates, 3) abnormal increases in extracorporeal venous pressure, 4) extracorporeal recirculation, 5) hemostatic disturbance after decannulation, 6) accidents with catheters inserted into the central vein, 7) skin irritation from antiseptics or plasters, 8) decannulation of needles during hemodialysis treatment. In this article we briefly outline clinical conditions associated with VA related accidents and their prevention. VA related accidents irritate patients and hinder efficient dialytic therapy. We must improve communication between dialysis patients and medical professionals to provide prevention, early detection and rapid treatment of shunt stenosis which is often the cause of VA related accidents.
