[ Summary ] We elucidate the causes of serious medical incidents which have occurred in dialysis facilities. Safety management procedures, particular to dialysis facilities, are required. There are many facets and reasons for these safety procedures. A primary factor is that dialysis is an invasive procedure, involving vascular access punctures. Another risk factor is repeated extracorporeal circulation. Further,dialysis medications are often changedas are the devices employed. In addition,renal failure patients often experience complications. The fact that dialysis procedures are conducted on many patients simultaneously may also be an associated risk factor. Many health services must be provided for these patients. Reporting systems to recognize and act on risk factors are necessary for basic health management safety.
