[ Summary ] Renal replacement therapy should be continued over long periods. In order to improve the outcome for dialysis patients, suitable dialytic modalities must be chosen for individual cases. Peritoneal dialysis (PD) is excellent in terms of small-molecule clearance, however, hemodialysis (HD) is superior to PD in terms of mid-size molecule clearance. Moreover, many PD patients remain in over-hydrated states. Preservation of residual renal function is important to correct these PD states. Icodextrin may be useful to maintain adequate hydration. In addition, combined PD / HD treatment is a feasible strategy to achieve targeted solute clearances and to improve certain clinical conditions.
