[ Summary ] An imbalance in the prevalence of renal replacement therapy is one of the most important health related problems in Japan. Until the present, most patients with end stage renal failure have been treated with hemodialysis. University hospitals should promote integrated renal replacement therapy to improve the overemphasis on hemodialysis. Establishment of educational systems for doctors, nurses and paramedics is important to understand the advantages and disadvantages of all three renal replacement therapies, leading to the development of integrated renal replacement therapy. In order to promote CAPD and transplantation university hospitals and educational facilities should teach all three kinds of therapies to their medical students and residents as well as young doctors who want to become specialists in this field.
