[ Summary ] At the time of dialysis initiation, all patients should be evaluated for the presence of peripheral arterial disease (PAD) with methods, such as ankle brachial index (ABI) to reveal possible asymptomatic PAD. Imaging studies should be undertaken for patients with severe claudication or critical leg ischemia who will experience revascularization.
Patients with PAD should be treated first by using smoking cessation therapy, medication, and supervised exercise regimens. These therapies may improve cardiovascular disease and provide patients with better prognoses. Revascularization procedures include endovascular therapy (EVT) and bypass surgery. The efficacy of EVT, especially for dialysis patients, has yet to be established. Inappropriate EVT may worsen symptoms in dialysis patients.
In conclusion, appropriate treatment for PAD should be customized for individual dialysis patients.
