[ Summary ] The goal of chronic kidney disease programs includes retarding the progression of CKD, prevention and treatment of CKD associated complications, and maximally improving quality of life and long term survival by providing adequate dialysis treatment. Nephrologists play a pivotal role in developing and implementing long term treatment plans for multidisciplinary CKD programs. Primary care physicians, renal dieticians, nephrology nurses and medical social workers are all involved in these programs. Patients' understanding of the disease and adherence to multiple treatment plans are key factors in the success of treatment. Continuous patient education through multiple channels is essential. Establishing effective, cooperative relationships between nephrologists and primary care physicians is also crucial.
