[ Summary ] Chronic kidney disease (CKD) is defined by structural and functional abnormalities in the kidney for more than 3 months, with or without decreased GFR, or by decreased GFR under 60 ml / min / 1.73 m2 for more than 3 months. The Japanese Society of Nephrology recommends that GFR be calculated by using IDMS-traceable MDRD Study equation, modified by a Japanese coefficient. Problems with the diagnostic method for CKD and application of the definition to patients with CKD may include minute calibration of autoanalyzers for serum creatinine, inaccuracy of estimated GFR using the modified MDRD study equation, disregard of proteinuria for risk stratification of CKD, inadequate classification of the stuge, particularly, stage 3, and insufficient evaluation of risk for CVD in Japanese patients with CKD. These problems need to be alleviated in the future. These difficulties may be eliminated by creation of the new equation for estimated GFR for Japanese , which is currently under way.
