[ Summary ] Diabetic nephropathy is a leading cause of end-stage renal disease (ESRD), accounting for 41.3 % of all new cases requiring dialysis therapy in Japan. Moreover, the prognosis for diabetic dialysis patients is poor as compared to non-diabetic patients. However, a recent study showed that good glycemic control resulted in better survival in diabetic patients on dialysis. Exogenous insulin can be used safely to manage glycemic levels in diabetic patients with renal failure and on dialysis. It is necessary to measure not only HbA1c and / or glycoalbumin but also blood sugar levels to evaluate glycemic control.
