[ Summary ] The guidelines of the Japanese Society of Nephrology concerning dietary management of chronic renal insufficiency in children were reviewed. We must consider dietary management of pediatric patients from the aspects of both renal prognosis and growth and development. There is no definite evidence but, some randomized control trials have shown no significant effects from protein restriction in terms of renal prognosis. Therefore, considering the potential harm to growth and development, protein restriction should not be considered necessary for infants. Protein intake control for peritoneal dialysis patients seems to be called for. However, energy intake should be increased because several reports have shown that aggressive nutritional support improved pediatric patients' growth and neurological development.
