[ Summary ] The ideal vascular access for patients undergoing hemodialysis is an arteriovenous fistula (AVF) in the forearm. However, with the recent increase in the incidence of diabetes mellitus and the number of elderly patients, several problems have arisen. These problems include (1) early dysfunction of AVF, (2) prolonged hospitalization due to temporary catheter insertion, and (3) postoperative steal syndrome. The solutions to these problems include (1) evaluation of the vessels using ultrasonography, (2) strategies for AVF creation in accordance with the patient's condition or prognosis, (3) postoperative monitoring and early detection of venous stenosis. It is necessary we establish a guideline for vascular access.
