[ Summary ] With the increasing number of patients having diabetic renal failure and requiring dialysis, more attention has been paid to the appropriate selection of dialysis modalities for diabetic patients with terminal stage renal failure. At present in Japan, the rate of hemodialysis (HD) is estimated to be 96.2 %, whereas that for peritoneal dialysis (PD) is approximately 3.8 %. Both dialysis modalities are incomplete to fully replace lost renal functions, having substantial advantages and disadvantages. PD first, in which we can utilize the benefits of residual renal functioning to their greatest degree. It is indicated for patients who are either diabetic or non-diabetic. PD is especially beneficial to stabilize blood pressure, yet detrimental to the control of blood sugar in diabetics. Appropriate modality selection must be presented for such patients on the basis of decisions, on which they are fully informed concerning HD and PD.
