[ Summary ] Idiopathic adynamic bone disease (ABD) in dialysis patients is characterized by low serum parathyroid hormone (PTH) concentrations. We performed histomorphometric analysis on bone specimens from biopsies done on patients with primary hypoparathyroidism and on autopsy specimens from bones of patients with secondary hypoparathyroidism related to longterm hemodialysis. Histomorphometric analysis indicated a diagnosis of ABD. Humplike structures, which have been called "minimodeling" have been revealed.
Bone formation mechanisms from minimodeling might contribute to preserving bone volume in dialysis patients with hypoparathyroidism, even in the absence of remodeling stimulated by PTH.
