[ Summary ] Three meta-analyses have demonstrated that low protein diets (LPD) have protective effects on the progression of chronic renal failure. Energy intake should be kept between 30 and 35 kcal/kg/day in order to avoid malnutrition. LPD is not associated with poor survival rates after initiation of hemodialysis. Supplemented very low protein diets ameliorate responsiveness to erythropoietin in predialysis renal failure. The JAPAN-KD study examines the effects of LPD on the progression of chronic renal failure in patients on ACE inhibitors and/or ARBs. The feasibility study demonstrates that LPD could besuccessfully administered in a multi-center study. A full scale study will begin soon.
