[ Summary ] In last few years, the fixed reimbursement system has been adopted to reduce increasing medical costs for dialysis therapy in Japan. The original concept of the fixed reimbursement system was to create competition between institutions in order to provide medical technology and services. Differing wildly from this original concept, it has been performed only to provide reforms of clinical economy. With this in mind, we must create a new system providing well balanced medical quality and quantity and also to reduce medical costs for dialysis therapy. In order to achieve this aim we must promote renal transplantation and peritoneal dialysis, to balance regional variations and to build a stable consensus on, the commencement and discontinuance of dialysis therapy for high risk patients. Problems arising from the fixed reimbursement system indicate future directions for therapy for end-stage renal disease in Japan.
