[ Summary ] The rate of utilization of peritoneal dialysis (PD) in ESRD treatment is lower in Japan compared with the average in developing countries. The most important point is lack of knowledge about the advantages of the therapeutic effects of PD by nephrologists in Japan. In spite of the rapid development of instruments for PD, the development of PD solutions has been extremely as slow, and has lead to problems with peritoneal membrane conditions. Although the recent availability of new solutions is expected to have the potential to protect or delay membrane damage, long-term clinical studies are required. The most serious complication with PD has been encapsulating peritoneal sclerosis. In spite of an incomplete understanding of the pathophysiological mechanisms of this state of morbidity, several risk factors and therapeutic strategies have been drawn from clinical observations.
